*  $300 deduction for prescription drugs, then $10/25/50 co-pay, deductible waived for generic

*  80/20% co-pay after deductible is met
*  $2,500 deductible single, $5,000 family, pay for all office visits
*  80/20% co-pay on major medical up to $4,000 out-of-pocked for single; $8,000 for family
